Image Comics provides this synopsis for Lost Fantasy: A hidden magical world exists beneath our own, first revealed over a century ago after a cataclysmic event. This breach unleashed monsters, leading to the creation of the Great Hunters, elite monster slayers who collaborate with global leaders to protect humanity. However, a recent massacre in Montana signals a significant shift, threatening both worlds. Rookie monster hunter Henry Blackheart must confront this new threat.

Pires states, "I'm thrilled to return to Image with Lost Fantasy, a blend of Western comics like East of West and Something is Killing the Children, combined with the immersive world-building and action of the JRPGs I loved growing up." He cites Final Fantasy's influence, particularly Tetsuya Nomura's character designs, as inspiration for the protagonist's appearance. He also acknowledges a subtle homage to Hironobu Sakaguchi's Lost Odyssey.
